2011 Most frequently asked Questions
Q: I was just wondering about how to go about making payment plan arrangements for winter retreat. With 2 students in the youth group now, It’s not going to be feasible for us to pay everything up front.
A: We have a payment plan set up for all trips, all you have to do is complete the manual registration form and the payment plan doc. Turn these into the Student Ministry Office (SMO) along with your 20% down payment and you will be registered.
Payments are made directly to the Student Ministry Office and should be marked for that trip and students name.
Participants are responsible for making payments in a timely manner and reminder notices will not be made.
Q:.Do I need to come in and talk to you or can I just drop off a deposit and installments? Just need to know how to go about everything.
A:These docs can be emailed or picked up in the SMO. All payment plans will be confirmed with a signature that the participants agree these guidelines.
(See the payment plan form for more details)
Q: Do I need to complete a new Medical Information Form each year for my son/daughter?
A:If the Student Ministry already has a Medical Information Form on file that was completed/signed after April 2010 for your son/daughter, you do not need to complete a new one. Remember to contact Lynda Gilbert at lynda.gilbert@uchurch.tv if any family member’s email address or phone numbers (home or cell) change or if your medical insurance information changes.
Q: Do students need to sign a new Code of Conduct form each year?
A:Yes, the Code of Conduct is valid for one year (from Vision Night to the next year’s Vision Night), so to be current right now we need a new form signed on or after August 28, 2011.
Q: Who do I contact if I’m not receiving the weekly email or other emails from Uprising Student Ministry?
